Care at Home May Be More Affordable Than You Think
‍For individuals who qualify, government-funded programs may help cover certain home care services, making it possible to receive the support you need while remaining safely and comfortably at home. Visiting Care Plus works with eligible individuals who receive services through SWIRCA, VA Community Care Network, and Indiana Medicaid and Home - and Community-Based Services (HCBS) program - Pathways to Aging.

What Are Home- and Community-Based Services?
Indiana’s Home- and Community-Based Services programs are designed to help eligible individuals receive care and support in their own homes and communities instead of living in a nursing facility or other institutional setting.Depending on the individual’s needs and approved plan of care, services may include:• Attendant Careo Assistance with bathing and personal hygieneo Dressing and groomingo Eating and meal-related assistanceo Mobility and transferringo Other activities of daily living• Home and Community Assistanceo Assistance with everyday household needso Support with maintaining a safe home environmento Assistance that helps an individual remain independent at home

How Does the Funding Work?
Government-funded home care is not simply a reimbursement that families receive directly. An individual must first qualify for the applicable program and have services authorized through the appropriate state process.A care manager or other authorized professional helps determine the individual's needs and the services that may be included in their Plan of Care. Once services are approved, an enrolled provider can provide the authorized care.

Pathways for Aging

Indiana Pathways for Aging is a state-managed Medicaid program designed to help Hoosiers age 60 and older receive the care and support they need while remaining as independent as possible. Participants choose a health plan and are assigned a care coordinator who helps connect them with medical care, long-term support services, and community-based resources. The program is intended to help seniors age safely in their homes and communities rather than in institutional settings whenever possible.
