
Pathways for Aging
Indiana Pathways for Aging is a state-managed Medicaid program designed to help Hoosiers age 60 and older receive the care and support they need while remaining as independent as possible. Participants choose a health plan and are assigned a care coordinator who helps connect them with medical care, long-term support services, and community-based resources. The program is intended to help seniors age safely in their homes and communities rather than in institutional settings whenever possible.
